The three serum bile pigments in obstructive jaundice and hepatitis.

چکیده
Many workers have observed that in obstructive jaundice and hepatitis hyperbilirubinaemia is due mainly to a rise in those pigments which give a " direct " reaction in the van den Bergh test. Cole, Lathe, and Billing (1954) have shown that there are two water-soluble pigments which give this reaction. They appear to be chemically related to bilirubin, but their exact structure is still unknown and they have been temporarily called pigment I and pigment II. Pigment II is the more polar of the two and is the principal pigment found in bile. The metabolic relationship between the three pigments is not fully understood and it was hoped that a knowledge of the amounts of pigment I and pigment II in serum might give further information on this subject. Using the method described in the previous paper (Billing, 1955), the sera from 17 jaundiced patients have therefore been analysed for bilirubin, pigment I, and pigment II. The clinical diagnosis of obstructive jaundice does not necessarily mean that the obstruction of the bile ducts is complete and there is also the possibility of some additional liver pathology being present. The effect of bile duct obstruction on bile pigment metabolism can be more easily studied if the obstruction is produced experimentally under standardized conditions. An analysis has therefore been made of the sera from a small series of rats, whose bile ducts were tied, and the results obtained will be compared with those from the pathological

The Pathogenesis of Icterus
These experiments indicate that, in obstructive jaundice, the bile which escapes from the liver is absorbed by the hepatic capillaries and carried by the blood to the kidneys. The presence of a thoracic duct fistula influences in no way the development of icterus after total obstruction of the common bile duct. Bile pigments, sufficient to give a Salkowski test, may or may not appear in the lym...
متن کاملThe separation of serum pigments giving the direct and indirect van den Bergh reaction.
Forty years ago van den Bergh developed a method for the estimation of bilirubin in serum by combining it in alcoholic solution with freshly diazotized sulphanilic acid. Shortly afterwards van den Bergh and Muller (1916) noted that by the omission of alcohol they could distinguish two types of reaction. Pigments from bile and serum from cases of obstructive jaundice developed a colour in the ab...

Obstructive jaundice in the first months of life is a fairly frequent phenomenon. Approximately twothirds of these infants have atresia of the hepatic ducts, and about two-thirds of the remaining can be classified as 'neonatal hepatitis', 'giant cell hepatitis', or 'thick bile syndrome' (Craig and Landing, 1952; de Toni and Romano, 1962; Gellis, 1961). 'Thick bile syndrome' is defined as 'neona...

1. We have confirmed previous work which shows that the injection of bile increases the tone of the vagus nerve, and that this action can be abolished after the administration of atropine. 2. We have found that the amoun...
